Can Wegovy help liver health? What the MASH trial shows beyond weight loss

Information scope · General health information checked October 15, 2026. Discusses US approval and an international trial; it does not assert authorization in other countries. Not individual diagnosis or prescribing advice; no individual medical professional review.

Semaglutide improved liver inflammation and scarring in selected adults with MASH. This evidence points to benefits beyond weight, but does not apply automatically to simple fatty liver or every GLP-1 medicine.

Fatty liver and MASH are different

MASH, or metabolic dysfunction-associated steatohepatitis, involves liver fat together with inflammation and liver-cell injury. Fibrosis means scar tissue in the liver. Fatty liver on an ultrasound alone does not establish MASH or the fibrosis stage.[1][4]

Benefits observed at 72 weeks

Published in 2025, the ESSENCE phase 3 interim analysis included 800 adults with biopsy-confirmed MASH and F2 or F3 fibrosis at 72 weeks. This randomized, double-blind trial compared weekly semaglutide 2.4 mg with placebo; both groups received lifestyle care. Novo Nordisk funded the study.[2]

The figures below describe liver-biopsy responses in study groups, not an individual's chance of a cure or the effects of every GLP-1 medicine. Gastrointestinal adverse events were more common with semaglutide.[2]

What the US approval covers

On August 15, 2025, the US FDA granted accelerated approval to Wegovy injection for adults with noncirrhotic MASH and F2–F3 fibrosis. Approval rests on liver-tissue improvement; long-term clinical benefits such as fewer liver-related deaths or transplants require further confirmation. US approval does not establish the same authorization in Korea or elsewhere.[1][3]

What to review alongside weight

Bring recent blood tests, imaging, a medication list, alcohol intake and weight changes. Ask whether fibrosis assessment is needed and agree on follow-up tests instead of judging recovery from weight or liver enzymes alone. Continue balanced meals and activity; choose medication and doses with your clinician.[3][4]

  • Have I asked about fibrosis assessment and follow-up tests?
  • Have I reported current medicines and adverse effects?

Frequently asked questions

Should everyone with fatty liver take Wegovy?

No. Your condition may differ from the trial population. Diagnosis, fibrosis stage, other illnesses and local authorization require clinical assessment first.

Can I skip meal and exercise care if the medicine helps the liver?

No. Both trial groups received lifestyle care, and US approval includes dietary changes and increased activity. Discuss a suitable plan instead of severe food restriction.
