Using a GLP-1 injection: what to check before the first dose

Preparation and pen controls differ by product. Before the first dose, ask a clinician or pharmacist to teach you using your actual pen and follow its leaflet. This article is a general checklist, not a replacement for device instructions.

Match the product to your prescription

  • Compare the product name and strength on the box and pen with your prescription.
  • Check expiry, storage and solution appearance as directed by the leaflet.
  • Confirm the previous dose date and today’s prescribed amount.
  • Identify whether your device needs a separate needle or is a single-use pen.

Understand subcutaneous injection and site rotation

South Korean Wegovy information specifies subcutaneous use in the abdomen, thigh or upper arm, not a vein or muscle.[3] Semaglutide injection guidance recommends changing the injection spot each time.[4]

For another product, check its own permitted sites and instructions. If you are unsure about handling the pen, ask to demonstrate the steps yourself while a clinician observes.

Check device-specific steps, including completion signals

Whether you attach a needle, check a new pen, read a dose display or confirm completion depends on the device. Do not apply one “hold for this many seconds” rule to every pen. South Korean Wegovy and Mounjaro prefilled pens have different designs.[1][2]

If liquid leaks or you are unsure whether a dose was delivered, do not independently add another injection. Record the time and what happened, then contact your clinician or product support. Do not infer the delivered dose from remaining liquid.

After use, attend to hygiene and records

Do not share pens or needles. Do not reuse detachable needles; follow your device’s instructions for removal and storage. Keep used sharps in a puncture-resistant container and ask a pharmacist about local disposal arrangements.[4]

Record the product, actual date and time, prescribed dose, site and any difficulty. Keep a planned dose separate from a dose you actually took.

Can I follow a pen video from another country?

The same product name can refer to different pen presentations and operations. Compare the country, strength and device on your box with the video, and prioritize the leaflet supplied with your medicine. Needle preparation and completion checks in Spain's FlexTouch leaflet apply to that device.[5]

Can I use liquid left in the pen for another dose?

Do not create extra doses or split doses from leftover liquid. Follow the approved number of uses and your prescribed amount.

Do all GLP-1 pens use the same needle?

Devices differ in whether they need a separate needle and which needles fit. Do not transfer needle length or compatibility from another pen; check your leaflet and pharmacist.
