Why a maintenance plan matters after reaching your goal

Reaching a target weight does not automatically end treatment or daily care. Discuss continuation or stopping with your clinician, and use food, activity and weight records to review a maintenance plan.

What did the STEP 1 extension show?

The 2022 STEP 1 extension reported that participants regained, on average, about two-thirds of their previous weight loss during the year after semaglutide 2.4 mg and structured lifestyle intervention were stopped.[1]

This is a group average, not an inevitable individual outcome or a 67% increase in body weight. Both medication and structured lifestyle intervention ended, so it cannot be reduced to the effect of medication withdrawal alone. It does not directly predict outcomes for other products, doses or care settings.

Plan maintenance before deciding when to stop

NIDDK explains that weight may return after weight-management medication is stopped and that lasting nutrition and activity habits matter.[2] Discuss treatment duration with your clinician in light of effectiveness, adverse effects and health.

Ask about follow-up visits, health measures to track and what to do if weight changes after your goal. Using an app is not a reason to independently reduce, stop or restart medication.

Records to review in maintenance

  • Weights measured in comparable conditions, with dates.
  • Missed meals or days with unusual appetite.
  • Actual activity time and strength-training records.
  • Changes and questions to discuss at your next visit.

Records are for understanding, not judging yourself
